Please arrive at least 15 minutes prior to your appointment.  Co-payments are due at time of service.  If you have an unmet deductible, your healthcare plan has provided a negotiated patient responsibility due at time of service. You will receive an automated appointment reminder by email and text 1 week and 3 days prior to your appointment.  We recommend you confirm your appointment and begin your check-in process prior to arrival.  Missed appointments are subject to a $50.00 No Show Fee.  Cancellation requests received with less than 24 hour advance notice will be subject to a $50.00 late cancellation fee.
